Connie Sunderhaus
 

Connie Sunderhaus RN CCM has 34 years experience of clinical nursing in the operating room, home health care, rehabilitation and medical case management.

She has served as a home health care administrator, independent case manager and health care consultant in the areas of rehabilitation, disease management, and workers' compensation. She has provided orthopedic- surgical expertise, repetitive trauma, head injury file management. She has also provided continuing education regarding current case management practice developments.

She represents Case Management as a member of the national JCAHO Disease Specific Care Advisory committee. She is an officer of the Case Management Society of America and served as a past president of the local state chapter. Connie has co-authored articles and presentations for the case management and disease management and disease management communities, emphasizing the direct relationship between patient education and optimal health outcomes. In her role as Business Development for Careguide, she has successfully launched the Careguide program at hospitals in the mid-Atlantic and won grants for hospitals to provide Careguides to women with breast cancer.
